Abstract

The umbilical vein of the adult can easily be reopened and used for various diagnostic and therapeutic procedures such as hepato-portography, measurement of the portal pressure, chemotherapy in tumours of the liver, antibiotic therapy, for portal decompression, and for metabolic and anatomic studies. 46 catheterizations of the umbilical vein have been performed for the above mentioned purposes, chiefly for hepatography, chemotherapy and anatomic study. Excellent visualization of the portal branches in the angiographic phase, and intense homogeneous opacification of the liver parenchyma in the hepatographic phase was achieved by injection of contrast medium and use of a rapid film changer. Tumours and metastases were demonstrated as distinct defects in the homogenous liver parenchyma. Liver metastases were diagnosed with great accuracy. Following chemotherapy via the umbilical vein, reduction in the size of the liver, reduction in the size of the metastases and necroses around the metastases were demonstrated clinically, by hepatography and by post mortem examination. The embryonic intrahepatic porto-caval shunt was reconstructed in the corpses of the adults by reopening of the umbilical vein and the ductus venosus Arantii.
