Abstract

A middle aged man presented with glare in right eye. On slit lamp examination, he had lenticular opacity in both eyes emanating from central suture with pyramidal projection above the anterior capsular surface with radiating spokes simulating an octopus like configuration which was confirmed on anterior segment tomography. Hence we describe a special entity cataracta octopoda which is a combination of anterior pyramidal and sutural cataract. Posterior segment evaluation was unremarkable. His low vision was attributable to amblyopia developed due to lenticular opacity. However cataract surgery was offered to him in both eyes primarily to treat glare as well as an added benefit of improving some vision due to the aberrated lens with central opacity. He had gained few lines of vision with resolution of glare symptoms.
