INTRODUCTION: The approach in young women, the decision when should be the initiation of radiological routine, according to the Brazilian Society of Mastology and the Brazilian College of Radiology, at age 35 would be a baseline mammogram. However, in relation to a young woman with the desire to become pregnant, there is no consensus. There are those who suggest a routine mammogram before pregnancy. REPORT: C. M. F. L. , female, 24 years, menarche at 13 years, child I. Use of oral contraceptives for three consecutive years. Deny hypertension, diabetes mellitus or family history of breast cancer. Complained of painless, nodule palpable in the union of the lower quadrants of the left breast for about 12 months with rapidly growing breast during pregnancy. Physical examination of the breast describes asymmetrical breasts, volumetric increase of the left breast, palpable nodule occupying almost the entire body left gland, mobile, regular skin and armpit. Radiological examinations (mammography and ultrasound) in combination showed a nodular image rounded, no microcalcifications, circumscribed, hypoechoic, heterogeneous areas without liquid, lymph node without abnormalities. DISCUSSION: mammography could be suggested in the pre-pregnancy tests, involving the breast ultrasound, thus avoiding disturbances during pregnancy in the case of suspicious findings.
