Abstract

We report the case of a healthy 26-year-old man with rupture of the sinus of Valsalva with dissection into the interventricular septum presented as ventricular fibrillation. Echocardiography showed precisely the ruptured site initiated from the right coronary cusp into the interventricular septal wall. That made a false cavity in the septum without outlet. There was moderate aortic regurgitation, but no significant dynamic left ventricular outflow tract obstruction. This finding was confirmed on chest computed tomography scan.
