Abstract

In Reply. —We thank Drs Waltz and Rubin for their comments regarding our article. The laboratory measurements of capsulectomy size were indeed direct anatomic measurements and the corneal magnification was not considered. We agree that the magnification caused by the cornea during a clinical operation should be considered before the preparation of the capsular opening. Most ophthalmologists consider this magnification factor when precise sizing of anterior segment structures is required. We did indeed note that nuclear expression is possible in association with a continuous circular capsulorhexis (CCC), without relaxing incisions. This observation, however, should not be taken as a recommendation for or against doing routine extracapsular cataract extraction without planned radial tears. Indeed, we suggest that the surgeon not hesitate to use one, or perhaps two, relaxing incisions during CCC and planned extracapsular cataract extraction, especially during the learning curve phase when making the transition to the CCC technique.
