Abstract

Therapeutic options for achalasia in elderly patients or those with multiple comorbidities includes endoscopic injection of botulinum toxin (Botox) at the gastroesophageal junction (GEJ) in a 4-quadrant fashion. It may, however, be difficult to inject Botox precisely into the muscularis propria because of the elevated resting pressures at the GEJ. We report a successful clinical outcome after cap-assisted Botox injection in a patient with achalasia.
