Abstract

Since 1993, these authors have used the free inferior gluteal artery perforator (IGAP) flap to reconstruct 35 breasts. The inferior buttock crease has proven to be the optimal donor site for the harvest of this flap. The scar is well-hidden in the natural buttock crease, and redundant skin and fat are removed, producing a shapely, lifted buttock. There have been no incidences of wound problems in the buttock crease, and no complications related to the sciatic nerve. The authors believe that the IGAP harvested at the inferior buttock crease is an excellent option for breast reconstruction and simultaneous buttock lift.
