Abstract
Health systems in today's world face challenges such as minimizing readmissions, improving quality of care, providing patients medication education, and increasing medication safety. One person or one department within a hospital can not solve these complicated issues alone. The key to overcoming these complex obstacles is collaboration both within a department and among multiple departments. "The only way to really move the dial on these issues is to work as a team," said Matthew Eberts, PharmD, MBA, Director of Pharmacy at the Western Pennsylvania Hospital, part of the West Penn Allegheny Health System.
