Abstract

Dear Sir, Bread and butter radiology We are well aware of the power of multislice computed tomography, but still occasionally get surprises. We recently reviewed the images of an elderly lady with abdominal pain and saw a small bowel intraluminal high-density lesion. It was clearly planar shape (square, circle, etc.) as it appeared linear in two planes (Fig. 1a,b). However, its true nature (bread-bag clip) was only shown with an oblique multiplanar reconstruction (Fig. 2). The surgeon removed the clip that had ‘pinched’ the mucosa (possibly seen in the images), necessitating a small bowel resection. (a,b) The foreign body appears linear in two planes. The true nature (bread-bag clip) is seen with an oblique multiplanar reconstruction. The clip appears to “pinch” the mucosa. The patient had no memory of swallowing this. It was probably recently ingested as some of the printing could still be appreciated on the clip. Unfortunately, the ‘expiry date’ was no longer visible!
