Abstract

The transthoracic esophagectomy with mediastinal lymph node cleaning has been a standard surgical method for patients with esophageal carcinoma recently. However, at present in Japan, the operative indication for blunt esophageal dissection (i.e., transhiatal esophagectomy: THE) is for cases with poor prognoses or mucosal cancer. We think that, in cases of carcinoma of the lower thoracic and abdominal esophagus, even when the prognosis is good, THE can reduce the operative stress compared with the conventional right thoracotomy.
