Abstract

RATIONALE: The objective of the study was the standardization of an extract of hazelnuts for its use in diagnostic and specific immunotherapy.METHODS: An extraction of the protein components of raw and toasted hazelnuts was performed. Afterwards it was characterized in order to know the amount and the quality of the most important allergenic components, Cor a 1 and Cor a 8. Sera of 30 patients with history of allergy to hazelnut, positive skin prick tests and DBPCFC with hazelnut extract were used.All patients underwent SPT performed according to standard procedure with raw and toasted hazelnut.RESULTS: A concentration of 2,69 mg/mL hazelnut extract was calculated to cause a wheal size similar to the histamine (10 mg/mL) one. For this raw hazelnut extract, 1 mg corresponded to 1,84 μg of the allergen Cor a 8 and 2,88 μg of the allergen Cor a 1. Most of the selected patients presented a IgE specific >3.5 UI/ mL.CONCLUSIONS: The characterized and calibrated “In house reference” (IHR) extract contains the main allergens, quantified in mass and it will serve as a pattern for the adjustment of standardized extracts of hazelnut used in diagnose and specific immunotherapy.
