Abstract

A 50-year-old man with chronic obstructive pulmonary disease and recent cataract surgery (postoperative week 2 in the right eye, and week 4 in the left eye) presented with painful vision loss in his right eye after a severe tussive episode. Examination confirmed endophthalmitis in the right eye (Fig A, light perception vision), and iris prolapse through the corneal incision in the left eye (Fig B, 20/20 vision). Urgent same-day pars plana vitrectomy with intravitreal antibiotics (right eye), and iris repositioning with corneal wound revision (left eye), were performed.
