Abstract

The buccal bifurcation cyst (BBC) is a rare inflammatory odontogenic cyst that was first reported by Stoneman and Worth in 1983, under the term “mandibular nfected buccal cyst-molar area.” The World Health Organization has included this lesion in the category of “paradental cyst” and has suggested the term “mandibular infected buccal cyst” for lesions that occur on the buccal surface of the permanent mandibular first molar in children 6 to 8 years old. Shear and Speight have suggested the term “juvenile paradental cyst” for the same entity. Pompura et al have argued for the term “mandibular buccal bifurcation cyst,” because the lesion is site and age specific. The BBC has specific clinical and radiographic features. The clinical features have included children ged 4 and 14 years, a usual association with a vital andibular first molar and, occasionally, the mandiblar second molar, deep periodontal pockets on the uccal aspect of the tooth, and an altered eruption attern of the associated tooth with buccal tilting of he crown and swelling in the affected molar area. The distinct radiographic findings are a U-shaped radiolucent lesion on the buccal tooth aspect covering the roots to a variable extent, an intact periodontal ligament space and lamina dura, an intact inferior border of mandible, tilting of the affected molars such
