Abstract

PACS is one element of an electronic triage system (ETS). Other elements under continuous development are MEDLINE, GRATEFULMED, RIS, HIS, telepathology, etc. There is ongoing research on artificial intelligence (expert systems), outcome probabilities, and practice guidelines as aids to diagnosis and treatment. Each research project and technological development is evolving independently of others, but to be cost effective in the clinical setting they need to be combined in an interactive system. An ETS, perhaps using PC-based computational agents such as knowbots, would integrate all data bases and contribute enormously to improving the capabilities of the diagnostician and the therapist. A review is made of the state of development of the numerous technologies, costs, and the time to diffuse ETS into clinical medicine. Gaps in knowledge must be filled, human resources identified, management systems determined, and the roles of the public and private sectors clarified.
