Abstract

A man working in pet food production developed urticaria, angioedema, rhinoconjunctivitis and acute asthma when inhaling dust of carob bean flour or other food dusts. Later, the same symptoms occurred after ingestion of peas and green beans. Allergy testing showed allergenspecific IgE antibodies and positive prick tests for pulse, flour and birch pollen crossreacting foods. Oral provocation tests with carob bean flour and soy flour caused angioedema, rhinoconjunctivitis and acute bronchial obstruction. Carob bean flour, soy flour, peas and green beans are part of the pulse family. Presumably, occupationally acquired sensitization to carob bean flour caused food intolerance also to soy flour, peas and green beans due to cross reactivity. Recognition as a job related illness is pending at present.
