Abstract

This is the first case report of auto-brewery syndrome (ABS) in a psychiatric setting. ABS, also known as gut fermentation disorder or endogenous ethanol fermentation, is a rare disorder where microbe overgrowth in the gut leads to alcohol fermentation, causing symptoms similar to alcohol intoxication without ingestion of alcohol. In this case report, we discuss possible etiology, clinical presentation, diagnosis, and management of ABS. This case will help practicing psychiatrists add ABS to their differential when seeing a patient in an emergency setting who appears intoxicated but denies any alcohol intake.
