Abstract
Percutaneous coronary intervention of chronic coronary artery occlusions (CTO PCI) has developed steadily over the past decade and is now accompanied by success rates of more than 85%. The training to become an independent CTO operator often takes several years and, in addition to the institutional requirements, requires a high level of personal commitment for ongoing further training. Randomized data showed symptomatic improvement but so far no reduction in mortality after CTO PCI. The right choice of patients, combined with a low peri-interventional complication rate, is all the more important. To ensure this, the German Cardiac Society (DGK) has decided to write a position paper for all cardiologists working in outpatient clinic and hospitals especially addressing all interventional cardiologists. A nationwide establishment of specialized CTO centers appears expedient. This approach will offer these highly complex procedures to the patient with a low and therefore justifiable risk.
