Abstract

Type II endoleak is the most enigmatic complication after endovascular aneurysm repair (EVAR). The best world experts on EVAR cannot agree on their significance, implications, and management. Despite hundreds of publications on the subject, there are still too many unanswered questions. Several aspects may be considered: (1) they are not per se a clinical problem, and they are very common; (2) they are generally innocent, leading to few events over time; (3) they have been associated with sac expansion and late rupture, which remains the most feared late complication of EVAR, but we cannot differentiate the dangerous from the harmless ones; (4) we have no good ways to prevent them from occurring or treat them when deemed necessary.
