Abstract

Abstract Six out of eight patients with tight oesophageal stricture associated with hiatus hernia and reflux were successfully treated by the Nissen-Rossetti operation in the chest. The surgical principle in preventing reflux was to increase the extrinsic pressure around the oesophagus by elevating a portion of the stomach through the hiatus and wrapping it around the lower oesophagus. The post-operative barium findings simulated those of a para-oesophageal hernia. However, as the cardio-oesophageal junction was left above the diaphragm, the thoracic portion of the stomach had to be filled with barium with the patient in the head-down position before post-operative reflux into the oesophagus could be excluded.
