He has highlighted an important aspect of our work, which indeed is outlined in the Methods section of our article. Our analysis was based on publicly available information regarding a radiologist’s primary practice address as recorded by CMS, not the full spectrum of locations at which the radiologist renders services. Our intent through this approach was to indicate where physicians are based, because if they are not locally based, they may need to offer teleradiologic services or arrange for some form of commuting arrangement, as Dr Larson’s group did. This requires a very special commitment to patient access through the development of an enabling practice infrastructure. However, such a conclusion may be misleading because of the method of localizing radiologists, which as described by the authors is based on “the zip code provided by CMS for each radiologist’s practice location.” Put another way, an entire group of radiologists billing the professional component may be attributed to only a single county location on the basis of their site of billing, regardless of where they actually live and practice.
