Abstract
The article presents a case report of a patient with cardioembolic ischemic stroke (IS), which was the first clinical manifestation of congenital atrial septal defect (ASD), asymptomatic until adulthood. The issues of anatomical classification, restructuring of intracardiac hemodynamics depending on the size and localization of ASD, its role in the development of IS are discussed. The diagnostic algorithm in patients with IS caused by paradoxical embolism is described, the current possibilities of ASD treatment and prevention of related disorders of cerebral circulation are discussed.

В статье представлено описание пациентки с кардиоэмболическим ишемическим инсультом (ИИ), развитие которого стало первым клиническим проявлением существующего дефекта межпредсердной перегородки (ДМПП), бессимптомно протекавшего до зрелого возраста. При проведении чреспищеводной эхокардиографии (ЧП-ЭхоКГ) обнаружен вторичный центральный ДМПП размером 4,1 мм, с легочно-системным соотношением кровотока (Qp/Qs) 1,5:1, без признаков расширения правых отделов сердца и легочной гипертензии Наряду с этим роль ДМПП в развитии преходящих и стойких нарушений мозгового кровообращения освещена недостаточно, что можно объяснить, с одной стороны, малой осведомленностью врачей о возможности формирования при данной нозологии право-левого кровотока, а с другой – недооценкой значимости этой аномалии в реализации ПЭ.
