•Pathologic features are crucial predictive factors for ypN0 in breast cancer patients after neoadjuvant chemotherapy (NACT) •Multiple sonographically abnormal nodes have been shown to be associated with increased nodal burden •Despite so, the initial number of sonographically abnormal nodes was not predictive of ypN0 •In patients for NACT, with many abnormal nodes on initial ultrasound, they should be given a trial of axillary preservation
