Abstract

To the Editor: We read with great interest the article concerning salvage banding for weight loss failure after Roux-en-Y gastric bypass (RYGB) in the May–June 2015 edition of SOARD [1]. First, we would like to express our appreciation of this very nice report describing a current problem for patients with weight regain after RYGB. Second, we would like to ask a few questions concerning the report. Regarding the endoscopic assessment of pouch and stoma size, the authors describe using an articulating measuring instrument via the working channel of the gastroscope. Is this a validated method for endoscopic gastric measurement? Which landmarks were used to ensure a standardized and reliable measurement for clinical and research purposes? Regarding the upper gastrointestinal contrast studies, we believe that the measurement of pouch size might not be reliable in cases of stoma dilation, because the contrast passes almost instantly through the stoma to the Roux limb, subsequently limiting the filling of the pouch,
