Abstract

The incidence of posterior cruciate ligament (PCL) injuries, reported to be from 1% to 40% of acute knee injuries, seems to be patient population dependent—3 % in the general population and 38% in regional trauma centers.1–10 The rate of PCL injuries in athletics is not specifically known. We see a 38% incidence of PCL injuries in acute knee injuries at our regional trauma center; 96.5% are PCL/multiple ligament injuries, and only 3.5% are isolated PCL injuries.9,10 The most frequently occurring are combined anterior cruciate ligament (ACL)/PCL injuries (45.9%) and combined PCL/posterolateral corner tears (41.2%). We have performed all of our PCL reconstructions using the transtibial tunnel technique. This chapter presents our indications, graft selection, surgical technique, postoperative rehabilitation program, and results of PCL reconstruction using the transtibial tunnel technique.
