Abstract

The article presents the results of clinical and imaging investigation and treatment of 16 patients with infraclinoid (37,5%) and supraclinoid (62,5%) aneurysms of internal carotid artery (ICA) that manifested as Tolosa – Hunt syndrome (THS). All the patients had oculomotor disorders as the result of III, IV, VI cranial nerves lesion. Pain of different severity due to the first brunch of trigeminal nerve lesion was seen in 11 (68,7%) patients. Exophthalmos was seen in one patient and chemosis – in two patients. The symptoms of total compression of the superior orbital fissure structures were seen in 4 (25%) patients. Giant aneurysms (diameter more than25 mm) were revealed in 3 cases (18,7%). In one case we revealed two aneurysms in one carotid system. Aneurysms ruptures were seen in 50% of cases. Spontaneous thrombosis was seen in 4 patients (25%). In 7 cases we made operative clipping of aneurysm necks with titanium clips through open access. Fatal cases or complications after surgical treatment were not revealed. The article presents three case reports with clinical features, treatment options and outcomes ofICAaneurysms that clinically masqueraded as THS.
