Abstract
肺癌術後に健側肺のみに出現した急性肺障害のため術後ARDSに陥った症例を経験した.肺癌術後にALIやARDS等の急性肺障害を発症する場合,その多くは術前から肺に線維性変化が認められることが多く,術後急性増悪として理解されている.術後急性増悪によるARDS症例では通常,肺障害は両側性に現れるが,自験例では手術操作を加えていない健側肺にだけ認められた.なぜ手術操作を加えていない健側肺にのみ急性肺障害が生じたのか原因不明である.術中片肺換気におけるdepending lungであるためventilator-induced lung injuryの関与が疑われるが推論の域を出ない.過去に健側肺のみに生じた肺癌術後急性肺障害に焦点をおいた報告はなく,急性肺障害発症のメカニズムを探求する上でも貴重な症例と考え報告する.
