Abstract

Laparoscopic radical hysterectomy using the Pune technique has been accepted as a standardized procedure due to its feasibility, duplicability and oncologic safety. We applied robotics to our technique to ten patients using the da Vinci Surgical robot. We used 5 ports including two assistant ports. Assistant surgeons provided clips, retraction and suction where necessary. The standard six steps included exposure of the pararectal spaces using the posterior ‘U’ cut, mobilization of the ureter, the anterior ‘U’ cut, ureteric tunnel dissection, lymph node dissection and low vaginal colopotomy. Specimen was extracted transvaginally.Average docking time was 10 mins. Mean operative time, blood loss, hosptital stay, margins and lymph nodes removed were comparable to our open series.There were no complications. We feel that the real advantage of robotics could be in patients with a high body mass index, a narrow pelvis, and when nerve preservation is sought.
