Abstract

Objective To investigate the applications and values of endoscopic hemoclip in diagnosis and therapy in pediatric surgery. Methods Between May 2002 and May 2011,totally 269 patients were diagnosed and treated using endoscopic hemoclips ligation, which included hemoclips in infant electronic endoscopes (Pentax EC3430/EG2540 and Olympus CF260/GIF260),Olympus MH-858 and MD-850 Ti alloy hemoclips,Olympus HX-6UR rotatable clip-fixing device, and biopsy forceps,as well as polyp recovery forceps. Results 889 clips in various devices were utilized in all patients (average 3.3 clips per patient),which included that 24 cases with gastrointestinal tract bleeding were treated (Mallory-Weiss syndrome in 3,peptic ulcer in 17,heterotopic pancreas in 2,and vascular malformation of gastrointestinal tract in 2); 209 cases gastrointestinal polyps were diagnosed and treated involving 196 cases of colonic polyps,6 cases of familiar polypsis coli,7 cases of Peutz-jegher's syndrome.Totally 289 polyps were removed completely using endoscopic hemoclips ligation (one off polypectomy in 157 cases and staged polypectomy in 52 cases whose diameter of polyp body≥2 cm or polyp neck≥1cm),and covered in one month after operation.Furthermore,36 cases of HSCR were also involved it,in which 17 cases underwent soave procedure. Conclusions The endoscopic hemoclip is one of the effective and safe tools in diagnosis and therapy for pediatric surgery. It can be used for treating gastrointestinal bleeding, polyps, gastrointestinal tract perforation, and is also helpful to diagnosis and treatment of HSCR.
