Abstract

Objective To explore the effectiveness of multidisciplinary team management mode in the nursing and treatment of patients with coronary heart disease (CHD). Methods Totally 151 CHD patients from the department of cardiology in a tertiary hospital from March to June 2015 were selected as the research object. 151 CHD patients were randomly divided into the experimental group and the control group. 77 patients in the control group only received the routine nursing and health education. 74 patients in the experimental group received the intervention of multidisciplinary team management for six months on the basis of routine nursing and health education. After the intervention, patients in two groups were investigated by chronic disease self-efficacy scales and self-management questionnaire of CHD patient, and patients′ self-efficacy level and self-management level were compared between two groups. Results The scores of self-efficacy and self-management in the experimental group were significantly higher than those in the control group (P<0.05). Conclusions The implementation of multidisciplinary team management mode intervention for CHD patients can enhance the self-efficacy of patients and improve their self-management ability.
