Abstract

Objective To study the clinical effect of lengthening donor renal vein using donor gonadal vein in right retroperitoneal laparoscopic living donor ephrectomy (RPLDN).Methods From August 2013 to April 2014,8 cases of retroperitoneal laparoscopic living right donor ephrectomy were performed at our hospital. Then the renal vein was lengthened using donor gonodalvein. The clinical data, including operation time, blood loss, warm ischemic time, length of gonodalvein, length of renal vein before and after prolonged and hospital stay, were analyzed retrospectively. Results Right RPLDN was performed successfully on all 8 cases without conversion to open surgery or apparent complications. The mean operation time was (88.0 ±7.5) min; mean blood loss was (50.6 ±6.6) ml; mean warm ischemic time was (75.8 ± 18.4) s; mean hospital stay was (4.2 ± 1.6) d,the length of renal vein was prolonged by mean (2.9 ± 0.9) cm.Kidney transplantation was performed successfully.Recipients' renal function returned to normal within 2-5 days.No complication such as venous thrombus was observed in the recipents during a 4-12 months follow-up.Conclusion Lengthening renal vein using gonodalvein during right RPLDN is a safe and effective procedure and worth being applied in clinic. Key words: Kidney transplantation; Right living donor; Retroperitoneal laparoscopic; Gonodal vein
