Abstract

Objective: To evaluate the application of inhaled nitric oxide (INO) in management of postoperative pulmonary hypertension in patients with congenital heart disease. Methods: 31 children who had refractory pulmonary hypertension or pulmonary hypertensive crisis after open hear operations for congenital heart defects received of INO. The changes of hemodynamics were observed at three different intervals. Results: The mean pulmonary artery pressure (MPAP) and Pp/Ps in children with refractory postoperative pulmonary hypertension or pulmonary hypertensive crisis after open-heart operations were significantly decreased after INO. The hemodynamics were significantly improved after INO. The effective rate was 87.1%. During INO, the levels of nitrogen dioxide (NO2) and Methemoglobin (MetHB) were found normal. Conclusion: Nitric oxide is and effective, safe, ideal pulmonary vasodilator. It plays an important role in the treatment of postoperative pulmonary hypertension or pulmonary hypertensive crisis in the patient with congenital heart disease.
