Abstract

Excision of congenital giant hairy nevi has been recommended because of their potential malignancy. From April 1997 to June 1998, we had applied artificial dermis, INTEGRA(superscript TM), combining with thin STSG for the surgical treatment of giant hairy nevus in two patients. The nevi was excised deep to fascia, the resultant wound was grafted with artificial dermis (INTEGRA(superscript TM)). Ultra-thin split-thickness graft (5/1000 inch) was harvested from back 2 weeks later and was grafted on the synthesized neodermis. The re-epithelization of donor site was completed within 10 days and without hypertrophic scar. We compared the results between INTEGRA(superscript TM) plus thin STSG and thick STSG alone in management of the same nevus. There is better skin pliability with INTEGRA(superscript TM) combining thin STSG. No gross difference of scar pigmentation, vascularity and height between two methods. INTEGRA(superscript TM) combining ultra-thin STSG can be an option for resurfacing large skin defect after excision of giant hairy nevus.
