Abstract

Objective To investigate the method and effects of nurse-led chronic disease management clinic in patients with cardiovascular disease. Methods A total of 300 patients admitted to the nurse-led chronic disease management clinic for cardiovascular disease of West China Hospital, Sichuan University from January to June 2016 were selected by convenience sampling and surveyed using self-designed questionnaire to investigate the satisfaction of patients. The nurse-led clinic volume and revisiting volume were collected in the first half year of 2015 and 2016 respectively. The effects of nurse-led clinic were evaluated with both revisiting and satisfaction rate. Results The revisiting rate in the first half year of 2016 was 51.9%, while in the first half year of 2015 was 33.8%, and there was a significant difference between the two periods (χ2=5.58, P<0.05) . Patients had a higher satisfaction rate of 95.7% for the chronic disease management in clinic. The two aspects with lower satisfaction rate were disease surveillance and emotion management. Conclusions Patients are satisfied with the chronic disease management in nurse-led clinic. Management of cardiovascular chronic disease clinic can effectively strengthen the management of patients with chronic disease.
