Abstract

A 38-year-old woman, gravida 3, para 1, presented to the emergency room with severe lower-abdominal pain at 8 weeks of pregnancy. Transvaginal ultrasonography showed a gestational sac located at the right uterine angle (Fig. 1A), with a surrounding 4.6-mm-thick myometrium. Laparoscopy revealed a 6-cm bulge rising from the right angle of the uterus (Fig. 1B) and medial to the uterotubal junction (Fig. 1C). Hysteroscopic evaluation showed that the gestational tissue was accessible through the uterine cavity. Therefore, after cervical dilatation with a Hegar dilator, the pregnancy was removed with suctional curettage under laparoscopic surveillance. Postoperative hysteroscopic examination confirmed that no residual tissue remained within the uterine cavity. Laparoscopy revealed immediate shrinkage of the bulging mass (Fig. 1D). The patient recovered uneventfully thereafter.
