Abstract

To investigate the characteristics of plantar pressure in elderly diabetic patients with or without diabetic peripheral neuropathy (DPN), 56 elderly (above 60 years) subjects were recruited: 19 diabetics with DPN (DPN group), 17 diabetics without DPN (D group) and 20 healthy subjects (H group). In-shoe dynamic plantar pressure during walking was measured. Statistical differences of peak pressure and proportion of high-pressure (above 200 KPa) area (%HP) between groups were analyzed. The peak pressures in inner and medial forefoot in DPN were significantly higher than that in D and H. The %HP in DPN was significantly larger than D in lateral heel, and larger than H in both inner forefoot and lateral heel. No significant differences were found between D and H. In elderly diabetic patients with DPN, the plantar pressure and high-pressure area increased significantly, especially in forefoot region. In the prevention of diabetic foot in elderly diabetic patients, attention should be paid to offloading of the forefoot region to reduce the risk of foot ulcers.
