Abstract
A theatre team from the Royal Berkshire Hospital recently spent two weeks operating in north-west India, close to the Tibet border. To reach the clinic in the Spiti Valley, it is necessary to cross the lower Himalayas via the Rohtang Pass (altitude 3978 metres) and Kunzum Pass (altitude 4547 metres). Much of the route is unsurfaced and somewhat tortuous: the usual services associated with European motoring are absent, as is mobile phone coverage. At approximately 4pm, 40 kilometres from the nearest habitation, the diesel engine of our minibus suddenly stopped as a result of a rock penetrating the primary fuel filter while travelling over the potholed mountain passes.
