Abstract

Purpose: Pediatric respiratory illnesses are the main causes of pediatric hospitalizations in the United States, resulting in significant morbidity and costs. Despite American Academy of Pediatrics recommendations against obtaining chest x-rays (CXRs) routinely for bronchiolitis, variation in practice and CXR over-utilization persists. Clinical practice guidelines (CPG) have had mixed success in reducing and sustaining unnecessary CXR utilization. Reliability theory and the use of high reliability interventions may improve the probability of successful guideline implementation.
