Abstract

A proliferating trichilemmal cyst (PTC) and a trichilemmal carcinoma (TC), which are both derived from the outer root sheath, were examined using light and electron microscopes to elucidate their morphological differences. Histologically, the characteristics of the tumor cells in the PTC were similar to those of the follicular isthmus and infundibulum. Tumor cells in the TC were similar to those of the lower portion of the outer root sheath. Ultrastructurally, the manner of keratinization in the PTC was similar to that of the follicular isthmus, while the manner of keratinization was not observed in the TC. From these findings, it is suggested that other indications, e.g. cytokeratins, which react only with outer root sheath cells, must be required to make the definitive diagnosis of TC.
