Abstract
症例は73歳男性．呂律障害，体動困難・意識障害で救急搬送となった．意識障害の原因は著明な低血糖（14 mg/dl）であり，ブドウ糖静注にて意識障害は改善した．CTにて大量の胸腹水，著明な肝萎縮，膵尾部に多発囊胞を認めた．肝硬変に伴う肝性胸腹水と膵癌に伴う癌性胸腹水の合併が疑われた．全身状態は徐々に悪化し第12病日に死亡した．剖検の結果，浸潤性膵癌を認めた．肝臓は肉眼的には褐色調で著明に萎縮し線維化は認めなかった．肝細胞にリポフスチンの沈着を認め褐色萎縮と診断した．
