Abstract
症例は80代，女性．肛門からの脱出を主訴に近医にて直腸脱と診断，Gant-Miwa-Thiersch手術を施行された．手術中に下部直腸の亜全周性側方発育型腫瘍を認めたため，手術は中止となり，当センターに紹介された．怒責診断にて，肛門より約10cm脱出し，下部に亜全周性側方発育型腫瘍を合併する直腸を認めた．術前大腸内視鏡検査で腺腫または腺腫成分を伴う粘膜内癌と診断した．下部直腸腫瘍と直腸脱の両方の根治性を考えて，経会陰的アプローチで侵襲が少なく，腸管の全層切除ができるAltemeier法を施行した．切除腸管は15cm，腫瘍径は90×40mmであった．病理組織学的検査結果は，腺腫成分を伴う乳頭腺癌で，深達度はm，ly0，v0であった．術後経過は良好で，排便機能の増悪は認めなかった．術後8ヵ月後の現在，直腸の脱出を認めていない．下部直腸に側方発育型腫瘍を合併した直腸脱に対して施行したAltemeier手術が有効であることが示唆された．
