Abstract

Based on 10 cases with abnormally high alpha fetoprotein (AFP) levels during pregnancy the following combinations of the AFP concentrations in maternal serum and amniotic fluid seem to be possible in fetal distress: 1) High maternal serum and high amniotic fluid AFP in cases with chronic fetal distress or intrauterine death. 2) Normal serum and high amniotic fluid AFP in acute or early fetal distress in fetal meningomyelocele hydrocephalus or other conditions where fetal proteins could pass from cerebrospinal fluid into amniotic fluid. 3) High serum and normal amniotic fluid AFP would be anticipated in cases where fetomaternal transfusion occurs: fetal serum AFP are 300-600 times higher than the corresponding maternal levels at term. The significance of low AFP levels in both maternal serum and amniotic fluid remains to be elucidated.
