Abstract
In human reliability analysis, the basic unit of analysis is the human failure event (HFE). In probabilistic safety assessment (PSA), the HFE is defined as the subset of hardware faults that can be caused by a human error. In practice, this does not intersect with human factors approaches that attempt to define the HFE through a task analysis. This paper presents a seven-step guideline to ensure a task analysis completed as part of a human factors analysis decomposes to a level suitable for use as HFEs for use in a PSA.
