Abstract

Radioiodine-resistant metastases are relevant problem in the diagnosis and treatment of differentiated thyroid cancer. Theaim of the work was to perform complex cytological and immunocytochemical studies of the punctates of postoperative metastases of papillary thyroid cancer in order to determine the features of their cells, on the basis of which it is possible to create analgorithm for preoperative prediction of the effectiveness of radioiodine therapy. Cytological and immunocytochemical studieswere performed on the material of fine needle aspiration biopsies of 110 metastases of papillary thyroid carcinomas detected inthe postoperative period (85 radioiodine resistant and 25 radiosensitive metastases). Statistical data processing was performedusing the non-parametric Mana-Whitney test and the ӽ2test in Statistica.11. A statistically significant difference was foundat p < 0.001 between radioiodine-refractory and radioiodine-avid metastases in expression of thyroid peroxidase and thyroglobulin as a result of this work, the algorithm of preoperative prediction of radioiodine refractory of the thyroid papillary carcinoma was proposed for the first time on the basis of detection of cytological and immunocytochemical features of thyrocytesin punctates. Determination of expression of cytokeratin 17 in punctates of primary papillary carcinoma, cytological featuresof thyrocytes, expression of thyroid peroxidase and thyroglobulin in postoperative metastases, allow prediction of radioiodinerefractory and determination of adequate therapy in patients with papillary carcinoma.
