Abstract

The presentation of a microscopic polyangiitis (mPA) as adult respiratory distress syndrome (ARDS) with a severe intensive care course is rare. A 67-year-old man was admitted with bilateral pneumonia. His current medical history revealed myalgia and palpable efflorescences on the lower extremities. During the clinical course, he developed ARDS with the need for mechanical ventilation and renal failure with dialysis. The finding of mPA autoantibodies in laboratory tests and the result of a renal biopsy confirmed the diagnosis of mPA; thus, a specific therapy adapted to the disease severity could be initiated.
