Abstract
There is growing awareness among disease management (DM) providers that risk stratification is essential if DM programmes are to deliver care with accuracy and efficiency. At the third annual Disease Management Congress [Pasadena, California, US; February 1999], Dr Milton Schwartz from Aetna US Healthcare described how Aetna’s ‘Healthy Outlook Program’ uses risk stratification to tailor care based on individual need.
