Abstract
•We report a patient with adult-onset Alexander disease (AOAD) mimicking MSA-C. •Cerebellar ataxia, spasticity, bradykinesia and urinary disturbance were observed. •The patient showed striatal dopaminergic deficit on DAT SPECT. •We should recognize that AOAD can affect striatal dopaminergic systems.
