Abstract
A 38-year-old woman presented to the emergency department (ED) with a lesion of the left deltoid area 2 months after treatment with an intramuscular injection of combination diclofenac and B vitamins for migraine headaches. Initial erythema and itching progressed to a necrotic black eschar associated with localized tenderness (Figure 1).
