Abstract

Abstract: Surfactant depletion and dysfunction are significant lesions in the pathogenesis of adult respiratory distress syndrome (ARDS). Some investigators have suggested surfactant replacement as a therapy for ARDS. We have replaced surfactant using flexible bronchoscopy in three patients with severe ARDS. Bovine surfactant (Survanta®, Ross Laboratories, Columbus, OH) at a total dose of 4 ml/kg was administered to each subsegment after the wedge position was established. There were no complications and each procedure was completed in less than 20 minutes.
