Abstract

Abstract The management of acute scrotum in children requires rapid evaluation and emergent inguinoscrotal exploration if deemed necessary. The differentials can range from torsion testis to the incarcerated hernia. Torsion of the sac of a complete is one of the more uncommon causes of acute scrotum and diagnosis requires a high index of suspicion. Here we report an even rarer case of torsion of an intussuscepted hernia sac, which is only the third such case of its kind. It is very important for emergency room pediatricians and pediatric surgeons to be aware of this condition because distinguishing it from torsion of the testis is often impossible and the surgical findings may be confusing for the uninitiated operator.
