Abstract

Acute fatty liver of pregnancy is a rare, potentially fatal complication that occurs in the third trimester or early postpartum period.¹ We are reporting a case of,29 year old Primigravida with IVF conception with DCDA twin gestation who presented at 22 nd week of gestation with complains of abdominal pain and vomiting .Her Liver enzymes was raised. Ultrasound abdomen showed Acute Fatty changes in Liver and hepatomegaly, diagnosed as Acute Fatty Liver of pregnancy .Thereafter Gastroenterology opinion was taken .Patient was followed up antenatally, serial Liver function tests was done, her liver enzymes were normalised. Then at 33 week of gestation, she complained of leaking per vaginum. Emergency LSCS was done in view of DCDA Twin with Preterm Premature Rupture of Membrane .Both twin babies were shifted to NICU for Preterm management. The mother was stable postnatally. Liver function test was normal. Suture removal was done on post op day 7 and patient was discharged.
